A case of rehabilitation for an Ankle Plantar Contracture after the Lower Limb Lengthening Operation

Abstract
After both lower limb lengthening operation, a 22-year-old female patient had ankle contracture and muscle weakness in both leg. The dorsiflexion was 5¡Æ in right ankle and -10¡Æ in left ankle. On manual muscle testing, ankle dorsiflexor was Fair grade in the right and Fair plus grade in the left as well as plantarflexor was Fair grade in the right and Fair plus grade in the left. Initial gait analysis showed increased hip flexion, decreased initial knee flexion motion, decreased dorsiflexion, decreased push-off ankle power generation and decreased speed, step length and cadence. She was received rehabilitative exercise program which was consisted of ankle ROM exercise, stretching of plantarflexor muscles, strengthening exercise such as isometric and isotonic exercise of 4 directional muscles around ankle, and ankle proprioceptive exercise. In addition, we added isometric, isotonic exercise on knee and hip muscles but isokinetic exercise on only hip muscles. After 4-month rehabilitation, the contracture of both ankle improved up to dorsiflexion 10¡Æ and strength of both ankle dorsiflexor and plantarflexor increased. Follow-up gait analysis showed normalization of hip flexion motion, initial knee flexion motion, ankle dorsiflexion motion and push-off ankle power generation as well as speed, step length and cadence increased.
